Provides limited supplemental support services to fill a service gap that cannot be filled in any other manner. Eligible Individuals
. Adult family member, or another individual, who is an informal provider of in-home and community care to an adult over 60 or to a person with Alzheimer's disease. An individual over 55 providing care to a person with a developmental disability.

Eligibility Criteria
Care receiver must be unable to perform 2 or more activities of daily living. Caregiver must live in the home or provide a substantial amount of in-home care.
