 About Program

Provides information about resources that will help families in their caregiver roles.

Assists caregivers in locating services from a variety of private pay and voluntary agencies.

.
Offers ongoing caregiver education and training opportunities.

 

Facilitates counseling and peer support to help caregivers cope with the stress of care giving and the disabling effects of a family member’s chronic condition.

Provides limited supplemental support services to fill a service gap that cannot be filled in any other manner.

  Eligible Individuals

.
Adult family member, or another individual, who is an informal provider of in-home and community care to an adult over 60 or to a person with Alzheimer's disease. An individual over 55 providing care to a person with a developmental disability.

  Eligibility Criteria

.

Care receiver must be unable to perform 2 or more activities of daily living. Caregiver must live in the home or provide a substantial amount of in-home care.
